Visiting Nurses Home Care (VNHC) is a New York State-licensed home care agency that has been providing professional home care services for Capital Region residents since 1988. These services include personal care aides, home health aides, licensed practical nurses, and registered nurses.

You do not need a physician’s referral to begin receiving services through VNHC. Home care may be covered by insurance or can be privately paid. To arrange for services, contact our main office at 518-694-9907.

After contacting us, our coordinator will then arrange for coverage by matching your needs to the most qualified health care professional. Service is provided on an hourly basis for most skill levels. Our nursing supervisor will then arrange to meet with you in your home to develop a plan of care most suited to your needs. Your physician will be contacted as necessary for further orders.

VNHC provides services to patients who reside in Albany, Clinton, Columbia, Delaware, Dutchess, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington counties.

Personal care aides (PCAs) assist with bathing, toileting, dressing, housekeeping, laundry, meal preparation, mobility, medication reminders, braces, splints and slings, and other non-medical tasks..

Companions/homemakers assist with meal preparation, companionship, light housekeeping, laundry, errands, and grocery shopping. They also can escort clients to appointments and activities and provide socialization and supervision.

Registered nurses (RNs) assess patient needs, coordinate services, case management, initiate care plans, make home visits, and supervise staff.

The Consumer Directed Personal Assistance Program (CDPAP) is a statewide Medicaid program that provides an alternative way of receiving home care services for chronically sick or physically disabled individuals with a medical need.

Consumers in this program have more control over who provides their care and how it is provided. The program provides help with activities of daily living and skilled nursing services. The consumer or a person acting on their behalf, called the designated representative, assumes full responsibility for the recruiting, hiring, training, and supervision of their personal assistant(s).

Eligibility for the program is determined by the New York Independent Assessor Program (NYIAP) and your local Department of Social Services or managed care organization. Visiting Nurses Home Care provides fiscal intermediary services for CDPAP. To get started with the NYIAP, call 855-222-8350. We thoroughly educate and familiarize all parties about how the program works. As the fiscal intermediary, VNHC coordinates processing of weekly payroll and associated benefits for the personal assistants.

Visiting Nurses Home Care is an approved provider for Service Coordination and Home and Community Support Services (HCSS) for the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Medicaid Waiver Programs.

The NHTD Program was created to enable New Yorkers with disabilities and senior citizens to live in the community instead of a nursing home. The TBI program provides services to persons with a traumatic brain injury and helps them live in the community setting of their choice.

The New York State Department of Health oversees these programs and contracts with Regional Resource Development Centers (contact information below) to manage the program throughout the state. The RRDC staff can explain the program and help individuals get started, as well as assist them in selecting a service coordination agency. Visiting Nurses Home Care is a service coordination agency.

Visiting Nurses Home Care (VNHC) is a private duty nursing service licensed by the New York State Department of Health to provide skilled nursing, home health, and personal care services to clients in 18 counties of northeastern New York.

We are a sister corporation of VNA Home Health, a certified home health agency which has served the needs of the Capital Region since 1880.

VNHC was founded to complement the care delivered by Visiting Nurses Home Health and other health care providers. Our services are supplied in your home or on a private-duty basis for patients in a hospital or nursing home. VNHC also provides services under the Consumer Directed Personal Assistance Program (CDPAP) and Nursing Home Transition and Diversion (NHTD)/Traumatic Brain Injury (TBI) programs, coordinates services for the NHTD/TBI programs, and provides care management services for individuals enrolled in specific managed long-term care programs.
