Requesting Medical Records
The best way to request medical records from a hospital in the Albany Med Health System is to use your MyChart account. Sign up now or log in.
If you do not go through MyChart, you can also fill out the form below. Be sure to download the form to sign and date it and email it to your hospital (as listed below).
Request Records of a Deceased or Incapacitated Patient
A family member or patient representative can request medical records for a deceased or incapacitated patient. To request records, you can:
Option 1
- Complete the Authorization to Disclose Medical Information form
- Obtain an original copy of the death certificate
- Obtain a power of attorney form (designating the requester as the POA)
Option 2
- Complete the Authorization to Disclose Medical Information form
- Complete a signed/notarized Affadavit (PDF)
- Obtain an original copy of the death certificate

Requesting a Change to your Medical Record
If you believe there is an error in your medical record, the first step should be to send a MyChart message to your provider explaining your concerns. If your provider agrees, they may edit the notes to correct the error. If your provider does not change the notes and you still feel that something is not correct, you may file a formal request to amend your record.
Patient Amendment Request Form (PDF)
The only person who can change information in your medical record is the author of the note. Health Information Management team members cannot. Therefore, we must get in touch with the author and request that they make the changes. You can expect an answer between 60 and 90 days. Once the amendment process has been completed, if you are unsatisfied with the outcome, you can contact the Albany Med Health System corporate compliance office or file a grievance with the Albany Med Health System patient relations department.

Note: Certified copies of birth and death certificates can be acquired at the vital records office in the city where the birth or death occurred.
