Saratoga Hospital Medical Group – Palliative Care sees patients on the inpatient units at Saratoga Hospital as well as in this outpatient setting in Wilton. Our health care professionals are highly committed to their patients, providing personalized, compassionate care. They work closely with a patient’s hospitalist, primary care provider, and physician specialist. The Palliative Care providers work alongside a team of Hospital and community professionals, including care managers, social workers, spiritual care providers, nurses, and others.

Your hospitalist can help you best determine if our inpatient palliative care services are right for you. Outpatient palliative care may be referred by hospitalist, primary care providers, and specialists. Patients are also welcome to self-refer. Outpatient care provides the same services as inpatient care with the addition of home visits, depending on patient eligibility.

Services include:

• Treatment and symptom relief
• Supportive care, including psychological, social, emotional, and spiritual support, and community resource referral
• Care planning and guidance with difficult and complex choices
• Home visits. Eligibility depends on the patient's mobility, level of illness severity, and access to transportation
