A medical home is more than a place. It’s an approach that puts your needs first, coordinates your care, and helps you stay as healthy as possible. As your medical home, we deliver care that meets the five requirements established by the U.S. Agency for Healthcare Research and Quality.
All Glens Falls Hospital primary care practices are currently certified by the NCQA (National Committee for Quality Assurance) as Level 3 Patient-Centered Medical Homes. In addition, we participate in a Primary Care First initiative with the Centers for Medicare & Medicaid Services and we are a member of the Adirondack ACO (Accountable Care Organization).

We are your partners in care. We work with you to manage your health and make sure decisions about your care reflect your needs, wants, and preferences. We also provide education and support so you can be an active participant in your health care.We take a team-based approach so we can meet most of your health care needs throughout your lifetime. Our team members provide prevention and wellness services, acute and chronic care, rehabilitation, and behavioral health care.
We coordinate your care with providers and organizations across the health care system, from specialists to hospitals and rehabilitation centers to home health care and community support service providers. Together, we help you avoid duplicate tests. Even more important, we work together to make sure you receive the right care in the right place at the right time.
We make it as easy as possible to get care and information when you need them. We have extended hours for certain services. We also offer 24/7 email or phone access.

Glens Falls Hospital is committed to providing primary care, specialty care, and other important health care services for our region. There are seven medical centers across the Glens Falls Hospital network, all conveniently located in communities in Warren, Washington, and Saratoga counties. In order to accommodate your busy lifestyle, most of our medical centers offer early morning/late evening or weekend hours.
