High-Risk Infant Follow Up Program
Supporting NICU Patients and Families After Discharge
The High-Risk Infant Follow Up program offers continuous support for neonatal intensive care unit (NICU) patients after discharge, who are at the highest risk for developing complex health and development issues during infancy and early childhood.
The goal is to identify concerns early and connect the family with the necessary support services, to help patients reach their fullest potential.

Offering Comprehensive Care
We strive to provide the top level of comprehensive care to our patients and their families.
The High-Risk Infant Follow Up team includes a neonatologist, a case manager, and a Neonatology fellow. They work alongside the patient's pediatrician and other subspecialists to ensure their optimal health and outcomes.
The team will assess the child's feeding, growth, development, thinking, language, and movement, help make referrals to specialists or support services, and serve as an advocate for the patient and family.
Patients seen in the High-Risk Infant Follow Up program will be referred to Developmental-Behavioral Health if there are ongoing developmental delays.
Depending on the age of the child, they may see a neonatologist and several other specialists at once, including nutrition, physical and/or occupational therapy, psychologists, or a case manager. We will work together to optimize your care.
The appointments may be an hour, or longer, depending on the care needed.
Please bring the feeding supplies your child uses (bottles, milk, formula, snacks, etc.) and have your child come hungry so we can assess their feeding and development skills.
